Our rehabilitation services are located in the Copeland Medical Center along with our primary care offices and The Village Pharmacy, so you can receive the services you need in one convenient place.

Become a Patient
ACV Health is accepting new patients for rehabilitation services. To become a patient, please have your hospital or physician fax the order or a referral form to our office.
For physical therapy services we are accepting patients who are at least 13 years old. For occupational and speech therapy we are accepting patients who are at least 18 years old.

INSURANCE
We are committed to making our services accessible to as many patients as possible.
For information regarding your particular insurance coverage, please call our office to discuss applicable copays, deductibles, and coinsurance. We accept most insurance plans and are happy to provide details on which insurance plans consider our services to be in-network as a courtesy. For more information, call 386-658-5865.
If you do not have insurance or have maxed out your benefits, give us a call to discuss a private payment plan.
Post-Surgical and Inpatient Rehabilitation
ACV Health care partner, Good Samaritan Center, at Advent Christian Village provides our patients with post-surgical and inpatient rehabilitation services.
