PRIMARY CARE REFERRALS
Copeland Clinic, at our state-of-the-art medical facility, offers a number of services and testing options for ACV Health patients and the surrounding community.
Services include primary care, x-rays, bloodwork, EKGs and laboratory services.

REHABILITATION REFERRALS
We are currently accepting patients for physical therapy services who are at least 13 years old. For occupational and speech therapy we are accepting patients who are at least 18 years old.

Advent Christian Village Home Care Agency is a fully licensed and Medicare-certified home care agency.
We provide care to adult patients over 18 years of age in Suwannee and Lafayette counties.

We are committed to making our services accessible to as many patients as possible.
For information regarding particular insurance coverage, please call our office to discuss applicable copays, deductibles, and coinsurance. We accept most insurance plans and are happy to provide details on which insurance plans consider our services to be in-network as a courtesy.
